Pre-conception weight loss is something I get asked about a lot, particularly in the Yukon where birth options have recently become more limited based on the full-term weight of pregnant mothers. The controversial “birth-weight-limit” issue is frustrating, as the weight cut-off does not consider height or body composition, or even the health of the pregnancy and fetus – which we know varies so much in each person and pregnancy.

This frustration aside, there is clear evidence-based value in working towards an individualized healthy weight/BMI before conception, and maintaining a healthy weight through pregnancy. New research has shown that pre-conception weight loss is particularly relevant to rates of natural conception, compared to those undergoing assisted reproduction (ovulation induction, IUI and IVF). This is likely due to ovulation impairments linked to PCOS (poly-cystic ovarian syndrome), which respond well to weight loss and insulin reductions through lower-carbohydrate diets as a primary intervention to promote regular ovulation.

Working with pre-conception patients to identify optional weight and body composition for healthy hormone production and ovulation will remain a priority for me, as well as maintaining health to avoid risk factors related to obesity in pregnancy (particularly gestational diabetes and hypertension).

But as always, it is important to operate with a lens of individualized assessment and care in all cases.
